To schedule an appointment
with a Doctor, Nurse Practitioner, Nurse, Counselor, or
the Health Educator please call SHCS at the numbers
above. Services are provided by appointment only. Please
allow 45 minutes to 1 hour for your office visit.
It is extremely important that
you arrive on time or you may be asked to be rescheduled
if you are late, however, we will make every effort to
accommodate you. Please call us 24 hours in advance if
you are planning to cancel your appointment, so that we
may accommodate another student.
All visits to the clinic are
confidential. We will not release any medical
information without your prior written consent. Your
confidentiality
is
important to us!
For the safety and protection
of your children and their health, we kindly request
that other
childcare arrangements are made for your appointment
time.
