OFFICIAL AND UNOFFICIAL       TRANSCRIPT REQUESTS   OFFICIAL TRANSCRIPT REQUESTS:   Students who have attended CSUSM may request official transcripts (no fee required) by sending a written request to:       Transcript Request Office of Registration and Records California State University San Marcos San Marcos, CA 92096-0001                Transcript Request Form   You must include the following information in your written request:   Campus ID Your full name and current address (include city, state, and zipcode) Your daytime phone number (with area code) Sign and date your request   Specific information regarding your request:   The full name and address (include city, state, and zipcode) to which the transcript(s) are to be sent The number of copies to send Date of your last attendance at CSUSM If your degree has not been posted, do you want us to hold it for degree posting?     UNOFFICIAL TRANSCRIPT REQUESTS:   Unofficial web transcripts are available on the SMART Web system.   Log in to SMART Web , select "Enrollment Services", select "Student Records", select "Academic Transcript", enter your information and submit your request. You may print this unofficial web transcript from your browser by selecting "File", "Print", select "Print".         Back to Registration and Records
